Kentucky IMPACT
Provides wrap-around services for children identified as severely emotionally disturbed who have been or are at risk for out-ofhome placement. The goal of Kentucky IMPACT (Interagency Mobilization for Progress in Adolescent and Children’s Treatment) is to try to avoid psychiatric hospitalizations by stabilizing the child and the family so that the child can remain in the home and community.

Transitioning
Provides wrap-around services for youth between the ages of 16 and 21 who have severe emotional disabilities. The target populations are youth who are likely to need ongoing mental health services into adulthood, or youth who have been in foster care and are transitioning out of the care of the Department for Community-Based Services. The goal of the Transitions Program is to prevent psychiatric hospitalizations by maintaining the youth in the community through support services, and to aid in a smooth transition to adulthood.

Custody Evaluations
Custody evaluations are available upon court order. The order should contain appointment dates and person responsible for payment.

Therapeutic Foster Homes
Pathways works with foster families and/or consultants to offer therapeutic services for children considered severely emotionally disturbed. The goal of the program is to work closely with the foster parents and/or child care provider to prevent disruption of the placement.

Family Preservation
Intensive in-home therapy and case management services are provided to families with children at imminent risk of removal from the home. In-home therapists spend an average of ten hours per week for approximately four to six weeks with each family in the program and are on-call 24 hours per day.

FACTS Program (Family and Children Together Safely)
This service assists families in keeping the family intact and achieving permanency for the children through provision of in-home therapy and communitybased prevention/intervention services as well as providing specific needs, such as clothing, shelter, etc. The duration of the services is a maximum of six months.

Family Reunification
Provides in-home and case management services to families being reunited after removal of the children. Services are provided up to six months in order to assist with the family’s transition and to increase the potential for successful reunification. The Family Reunification Program is available in the FIVCO and Gateway counties.

Family Advocacy Network
Parents United for Support and Help (PUSH) is a support group for parents of emotionally disturbed children in the FIVCO region. Pathways assists this network to provide peer support and training to parents of children with emotional disabilities so they can in turn provide peer support to other parents.
